Design the next Hutta Orthodontic Specialists t-shirt to win an iPod Touch and your design will be printed on all of our new t-shirts to be passed out at our Patient Appreciation Picnic on October 2, 2011 at Highbanks Metro Park!
Print out the form below, complete it and design the t-shirt template. Once completed, drop it off or mail it to either one of our office locations by August 31, 2011.
